Remove a device push subscription

/api/me/push-subscriptions/:id

Remove one of the acting user's push subscriptions (native logout / opt-out). The row must belong to the acting user (404 otherwise — another user's / tenant's subscription ids are never revealed). RLS keeps it tenant-scoped.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
