Add an MCP server (AI manager only)

/api/ai/mcp-servers

Saves the server and asks it for its tools (tools/list) at once. A server that does not answer is still saved, with the reason in last_error, so the address or token can be fixed and the list refreshed. The address must be public https. No tool reaches the model until it is approved (PUT /api/ai/mcp-servers/{mcpConnection}/tools). Throttled.
