Approve or withdraw an MCP server's tools (AI manager only)

Approval binds a tool as the server describes it now; a later change on the server's side withdraws it. side_effect marks a tool that changes something outside the conversation: the assistant asks the customer to confirm first and calls it at most once per reply. Tools not named are left as they are.
