Permanently delete a conversation (irreversible)

/api/conversations/:conversationId/permanent

Hard-deletes the conversation and EVERYTHING attached to it, leaving no orphans: its messages, attachments (rows AND stored files), notes, tags, participants and AI training examples. ai_usage and visitor_sessions are KEPT but de-linked (billing / analytics integrity must not change retroactively). Irreversible and distinct from the soft delete (trash). ADMINS ONLY.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
