Revoke any MCP key in the workspace (team managers)

Revokes a member's MCP key. Requires members.manage. A key of another workspace answers 404. Audited as mcp_key.revoked on the owner.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
