Built for a contact's card and widened for the diary screen. from/to select an OVERLAP rather than a start, so a treatment that began before the window and is still running inside it is in the answer — the long appointment at the top of a Monday, which a starts_at filter drops. Paginated: it used to take the first 200 rows and say nothing about the rest, which is the shape a busy month silently loses half of. meta.total is what lets a calendar say "1 200 of these, showing 500" instead of drawing holes.
