Records a manual grant/revoke (source admin_manual, stamped with the acting staff user as the audit actor) and appends an immutable audit event. Requires the contact update ability.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
