Create the site's ready-made lead form (admin)

Creates and publishes a contact form (name, phone, e-mail, message) that CREATES A CONTACT for every submission (matched by e-mail, then phone), and makes it the site's default form. The intake follows each form's own destination.create_contact; this is the form that has it on from the start. Audit row website.default_form_created.
